Types of Assessments
ADHD Evaluations
An ADHD evaluation looks beyond whether you sometimes feel distracted or disorganized. We explore attention, executive functioning, impulse control, activity level, emotional regulation, and how these patterns affect everyday life. Depending on your age and concerns, the evaluation may include interviews, attention and cognitive testing, questionnaires completed by you and people who know you well, and a review of relevant school, medical, or treatment records. We also consider other factors that can resemble or occur alongside ADHD, such as anxiety, depression, trauma, sleep difficulties, learning differences, and autism.
Autism Evaluations

Combined ADHD & Autism Evaluations
ADHD and autism can overlap in areas such as executive functioning, sensory experiences, social interaction, emotional regulation, and the need for predictability. A combined evaluation considers both possibilities within one integrated process rather than requiring you to pursue two separate evaluations. Testing is selected to help clarify which patterns are best explained by ADHD, autism, both, or another concern—and to translate that understanding into useful recommendations for daily life, school, work, relationships, or treatment.
Learning & Academic Evaluations
A learning evaluation examines how you take in, organize, and use information, with particular attention to reading, writing, and mathematics. It may include measures of cognitive abilities, memory, processing speed, executive functioning, and academic achievement, along with interviews and a review of educational history and records. The evaluation can help identify learning disorders such as dyslexia, dysgraphia, or dyscalculia, clarify why school or academic work has been difficult, and provide recommendations that may be useful when pursuing support or accommodations.
Diagnostic Clarification Evaluations
Sometimes the question is broader than one diagnosis. You may have received different explanations over time, notice symptoms that seem to overlap, or feel that an existing diagnosis does not fully capture your experience. A diagnostic clarification evaluation takes a wider look at emotional, behavioral, cognitive, developmental, and personality functioning. The specific measures are selected around your questions and may explore concerns involving anxiety, mood, trauma, obsessive-compulsive symptoms, personality patterns, attention, or other areas of functioning.
Focused or Targeted Evaluations
A focused evaluation may be appropriate when you have one clearly defined question and do not need a broader diagnostic assessment. This might involve updating an earlier diagnosis, evaluating a specific area of functioning, or supplementing recent testing that is already available. After reviewing your concerns and existing records, we’ll determine whether a targeted evaluation can adequately answer your question or whether a more comprehensive approach would be more useful.

What to Expect
1. Start with Your Questions
We begin with a brief consultation to discuss what you hope to understand and whether Forge is the right fit for your evaluation.
2. Build the Evaluation
Your evaluation is designed around those questions and may include interviews, questionnaires, records, collateral information, and one or more testing appointments.
3. Make Sense of the Results
After testing is complete, the information is considered together to identify meaningful patterns, strengths, and areas where additional support may help.
4. Plan What Comes Next
We’ll meet for a feedback session, and you’ll receive a written report explaining the findings, diagnostic conclusions when appropriate, and practical recommendations.

The appropriate evaluation and final fee will depend on the questions you would like answered, relevant history, complexity, and anticipated testing needs.
We’ll discuss the recommended scope and cost with you before formal testing begins.
Payment & Questions about Insurance
Forge Therapeutics is not paneled with insurance companies and does not bill insurance directly. Upon request, we can provide a superbill that you may submit to your insurance company for possible out-of-network reimbursement.
Coverage varies by plan, and reimbursement is not guaranteed. Clients are responsible for confirming their out-of-network benefits and for the full cost of the evaluation, regardless of whether reimbursement is provided. Payment due day by end of day, first day of in-person testing.
